Demolition Permit Submittal Items Checklist

The following items must be submitted at the time of application for a permit to demolish any principle structure or any portion of a principle structure totaling more than 1,500 square feet which would trigger the need to submit a Construction and Demolition Debris Diversion Plan.

  • Completed Application for Building Permit.
     
  • Proof of ownership (copy of property deed), and a written statement by the property owner detailing their intent and future use of the property.
     
  • Copy of contract between the contractor and owner for demolition.
     
  • List of contractors including demolition, electrical and plumbing.
     
  • Certificate of Insurance for demolition, electrical and plumbing contractors.
     
  • Copy of electrician’s license.
     
  • Copy of plumber’s license.
     
  • Plumber’s letter of intent, required for disconnection of water service. Separate permit required for service disconnection.
     
  • Asbestos Inspection Report, including the following information in addition to information currently mandated by state and federal regulations for asbestos inspection reports.
    • Sampling Methodology: A detailed description of the methodology used to determine sampling locations, ensuring a representative collection of samples from all suspect homogeneous areas.
    • Sample Area Identification & Scaled Drawing: Clear identification of each sample area, accompanied by a drawing of the building/area to scale with dimensions.
    • Precise Sample Locations: The scaled drawing must clearly and precisely indicate each sample location.
    • Contextual Photographs: Clear photographs of the material from which each sample was taken. Photographs must provide sufficient context by showing adjacent homogeneous surfaces.

      Or,
       
    • Certification from a licensed asbestos abatement contractor indicating the structure to be demolished is free of asbestos containing materials.
       
  • For projects where the structure to be demolished has more than 1,500 square feet of total gross floor area, a Construction and Demolition Debris Diversion Plan must be submitted. A copy of the plan may be obtained from the Building Department’s web page.
     
  • Plat of Survey.
     
  • Proposed Site Grading Plan.
     
  • Site Preparation Plan, see attached checklist.
     
  • If a septic system exists on the property, the applicant must submit a copy of the septic pumping bill indicating that the septic tank has been properly drained. An inspection must be scheduled prior to the issuance of a Demolition Permit to witness the cracking of the bottom of the septic tank after the tank has been pumped if not fully removed.
     
  • Utility Company verifications of disconnections must be submitted prior to the issuance of a permit to demolish any structure to which the utilities listed below have service connections.
    • Private Well - Lake County Health Department.
    • Septic tank - Lake County Health Department.
    • Gas - North Shore Gas.
    • Electric - Commonwealth Edison Company.
    • Water - Village Engineering Department. Separate permit required.
    • Sewer - Village Engineering Department. Separate permit required.
